What Is a DOT-SAP Assessment?
The Substance Abuse Professional (SAP) is a federally mandated evaluator who assesses employees who have violated DOT drug and alcohol regulations (49 CFR Part 40). If you test positive for drugs or alcohol, refuse a test, or otherwise violate DOT regulations, you are required to complete the SAP process before returning to safety-sensitive duties.

This process is essential for CDL truck drivers, airline employees, pipeline workers, transit operators, railroad employees, and other DOT-regulated safety-sensitive positions who have experienced a DOT violation.
1. Initial DOT-SAP Assessment
The process begins once you receive a DOT drug or alcohol violation (such as a positive test or refusal to test). At this point, you must stop performing all safety-sensitive duties and schedule an appointment with a qualified DOT-SAP professional in Maryland.

3. Follow-Up SAP Appointment & Return to Duty
Once you have successfully completed your recommended program, you’ll schedule a follow-up evaluation with your SAP professional. During this session, we will verify your compliance and issue your Return to Duty letter (cost $299.00), which will be sent to your employer.
For CDL some drivers in Maryland, this step also requires visiting the Maryland Department of Motor Vehicles (DMV) to reinstate your CDL license.
2. Education and Treatment Recommendations
Following your assessment, we will recommend the necessary alcohol or drug education, counseling, or treatment program required by DOT regulations. This may include:
A. Alcohol-Drug Education Training, a state-approved educational program that meets DOT requirements.
B. Substance use treatment program (outpatient or inpatient, depending on evaluation results).
Each program is designed to ensure that employees receive the appropriate level of care and intervention before returning to work.
4. Follow-Up Testing Requirements
Even after returning to work, DOT regulations require follow-up testing to ensure ongoing compliance. Your SAP professional will determine the follow-up testing plan, which includes a minimum of six random drug/alcohol tests in the first 12 months. Additional testing may be required for up to five years (60 months).
