Understanding the DOT-SAP Process

What Is a DOT-SAP Assessment?

The Substance Abuse Professional (SAP) is a federally mandated evaluator who assesses employees who have violated DOT drug and alcohol regulations (49 CFR Part 40). If you test positive for drugs or alcohol, refuse a test, or otherwise violate DOT regulations, you are required to complete the SAP process before returning to safety-sensitive duties.
A DOT-SAP professional will:

  • Conduct an initial assessment to evaluate your substance use and determine your required treatment or education program.

  • Recommend the necessary steps to comply with DOT regulations.

  • Monitor your progress throughout the process.

  • Complete a final follow-up assessment and provide a Return to Duty letter once you’ve met all requirements.

This process is essential for CDL truck drivers, airline employees, pipeline workers, transit operators, railroad employees, and other DOT-regulated safety-sensitive positions who have experienced a DOT violation.

Step-by-Step:
How the DOT-SAP Process Works

DOT-SAP Return-to-Duty Process: How IBH Helps You Get Back to Work

When a DOT drug or alcohol violation occurs, federal law requires you to complete the Return-to-Duty (RTD) process with a qualified Substance Abuse Professional (SAP) before resuming any safety-sensitive duties. IBH makes this process simple, fast, and fully compliant with 49 CFR Part 40.

Step 1 — Initial DOT-SAP Assessment

Once you receive a DOT violation—such as a positive drug or alcohol test, refusal to test, or another prohibited action—you must immediately stop performing safety-sensitive duties, as required by DOT regulations.

Your first step is scheduling a face-to-face SAP evaluation with a qualified DOT-SAP. IBH has multiple certified SAP providers in Maryland and offers quick, easy scheduling so you can begin the process without delay.

Cost: $199
Purpose:

  • Review your DOT violation

  • Assess your substance use history

  • Determine what education or treatment is required under federal rules

This assessment is the official starting point of your Return-to-Duty process.

Step 2 — Education & Treatment Recommendations

After your initial evaluation, your SAP will provide a personalized treatment or education plan, as required by DOT regulations. Your recommendations may include:

A. Alcohol & Drug Education Program

A state-approved educational course that satisfies DOT education requirements.

B. Substance Use Treatment Program

Levels of care vary depending on the clinical assessment and may include:

  • Outpatient counseling

  • Intensive outpatient treatment

  • Inpatient or residential treatment

Every plan is designed to ensure you receive the appropriate services to support a safe return to duty.

Step 3 — Follow-Up SAP Appointment & Return-to-Duty Authorization

Once you complete all required treatment or education, you’ll meet with your SAP again for your follow-up evaluation. During this appointment, your SAP will:

  • Verify successful completion of all recommendations

  • Determine whether you are ready to return to safety-sensitive work

  • Issue your Return-to-Duty Report/Letter

Cost of RTD Letter: $199
Your Return-to-Duty documentation is sent directly to your employer, as required.

CDL Drivers in Maryland

Certain Maryland CDL holders may also need to visit the Maryland MVA to complete reinstatement procedures after the SAP process is finished.

Step 4 — Follow-Up Testing Requirements

Even after returning to work, DOT regulations require ongoing follow-up testing to ensure continued compliance. Your SAP will create a customized, federally mandated follow-up testing schedule, which includes:

  • At least six (6) unannounced drug/alcohol tests in the first 12 months

  • Additional testing that may continue for up to five (5) years (60 months)

  • Tests conducted separately from normal random testing pools

Your employer is responsible for carrying out the testing schedule exactly as prescribed.
